Minnesota Nursing Home Short-Stay Experience Survey 2023

  Overview

The Minnesota Department of Human Services (DHS) is committed to achieving the highest quality care possible in Minnesota's nursing homes. To do so, they are asking consumers who have had a recent stay in a nursing home about their experience. The questionnaire asks for your opinions and helps DHS share information on their report card about the quality of nursing homes. The Minnesota Nursing Home Report Card with facility scores is available to the public at NH Report Card. They will also summarize the results of the survey and provide them to the nursing home to guide their quality improvement efforts.

Process

Please complete the survey and return it in the enclosed, pre-paid envelope. The survey will be mailed to Vital Research. Vital Research is an independent research firm, which maintains HIPAA and HITECH protocols that protect the privacy and confidentiality of discharged residents.
